The artwork was created by Todd McFarlane Entertainment, with pencils by Greg Capullo and inks by Todd McFarlane, both of whom are well-known comic book artists. The cover art depicts a young girl in a red dress playing hopscotch toward the edge of a cliff while a group of other children wait to follow her. The album title was suggested by Korn bassist Reginald "Fieldy" Arvizu, and it was intended as a taunt to other bands that were following in their musical style, rather than a reference to the children's game. This was the third consecutive Korn album to feature children in a disturbing context on the cover.
